Why Do I have to update my insurance card?

An HMO, or Health Maintenance Organization, (or Insurance Cards with a Designated PCP) is a type of managed care health insurance plan. These plans typically offer lower costs, but you will have a more restrictive provider network, and you will have to coordinate your medical care through a designated primary care physician (PCP). It requires a simple phone call to the member services line (found on the back of your insurance card), informing the insurance company of your recent change.  The instructions below will assist you in making this change. Once completed, a new card should arrive in the mail in approximately 7-10 days. Alternatively, most insurance companies have a member services portal where the new card can be downloaded. Please update us with this new card as soon as possible. 

Please Follow / Fill Out the Provided Steps Below:

1) Call the Member Services Phone Number on the back of your Health Insurance Card.

2) Once speaking with member support, inform them you need to update the PCP on your Policy.

3) Because Dr. Brock has multiple records (from practicing in the area at different locations), the correct record with the insurance company needs to be identified when updating your policy. To ensure this, please provide the Physician NPI Number & TIN# below to the call center agent:
