How Do I Become a New Patient?
Thank you for choosing Recover Health & Wellness for your medical needs! All patients not seen before at this clinic location (3100 W. State Highway 22, Corsicana TX. 75110) are considered a NEW patient. This also INCLUDES previous patients of Dr. Brock seen before at prior clinic locations.
Note: If you have been seen before at THIS clinic please go back and select the section titled Established Patients.
The process is simple and straight forward (Please read this section entirely before clicking the link):

- Fill Out The Required Information
- Take a Picture of Your Insurance Cards & Attach Them
- Click Submit
- Wait for the Clinic To Call You Back & Confirm!
- PCP Declaration / Update – If your insurance policy specifically lists the name of your doctor on the front of your card, please follow the instructions HERE prior to making your appointment request below. In order for us to bill your claim, Dr Brock at Recover Health & Wellness (must be this location) must be listed correctly.
Attention – Please Review The Items Below Carefully Before Continuing With An Appointment Request Below:
- Be sure to have a copy of all your Insurance Cards on hand as you will be asked to take detailed pictures of the front and back when submitting your request. Failure to submit your insurance cards online with your request will DENY your request!
- Recover Health & Wellness has adopted the use of technology and digital communication. All patient paperwork and appointment communications are handled via Text Messaging. Patients who do not posses a text capable cell phone MUST provide a Personal Email address that is monitored for appointment needs.
- At Recover Health & Wellness, we value our patient’s time by prioritizing the need to keep our clinic on schedule. To accomplish this, we REQUIRE new patients to submit appointment forms at least 36 HOURS in advance. Forms will be send via Text/Email within 7 days of upcoming appointments. Failure to do so will result in CANCELLATION.
